Photography and media consent form

Under the 2018 Data Protection Act, your rights include:

• Your consent (to the publication of yours or your child’s images) can be withdrawn at any time.
• Your images/recordings will not be used for any other purpose without your further consent.
• The contact information you have provided will only be used for a member of the Group to get in touch to discuss your images/recordings/comments.

Conditions of use

• We will not include personal email, postal addresses or telephone numbers on video, our website or in printed publications and advertising.
• We may use group images with very general labels such as ‘Customers at Housing Plus Group properties’.
• We may use the image in literature unrelated to when and where the picture was actually taken.
• We may include your full name with the image.
• We will retain your consent form for six years, however we reserve the right to store and retain the images/recordings in our archive indefinitely.
• These images may also be used by Housing Plus Group’s operating companies (Homes Plus, Care Plus, Severn Homes, Property Plus and County Town Homes).
